The purpose of this form is to borrow materials that are not available in the Madisonville Community College libraries. Charges will be limited to those made by the lending library.

Please make sure that the Madisonville Community College libraries do not have the material that you want to request. Please do not request materials just because they are presently checked out from our libraries.
